Assessment and Reviewing Officer (Community Reablement)

Job Purpose

MioCare’s Community Reablement Service offers planned, short-term intensive assistance to people in their own home, helping them to regain the skills needed to live as independently as possible and increase confidence in their own abilities.

As an Assessment and Reviewing Officer (ARO), you’ll support the Service Managers to deliver a high-quality service that meets the needs of people we support.

You’ll work with people we support, carers and other professionals in completing assessments, reviewing, monitoring and recording progress on reablement packages.

You’ll assist the management team in ensuring the expectations of commissioners and the Care Quality Commission (CQC) are met through compliance with policies and procedures and excellent professional standards.

You’ll actively alert and report any issues or cause for concern to your line manager, and will provide an environment where both people we support and colleagues can thrive.

From £16.23 per hour

Location

Based at Spindles Town Square Shopping Centre (OL1 1HD) – but with a necessity to be able to travel across the borough of Oldham.

Key Responsibilities

To work with people we support, carers and other professionals in completing assessments, reviewing, monitoring and recording progress on reablement packages.

To monitor and review relevant paperwork pertinent to individual customers i.e. medication recording charts, support plans, finance logs etc. To maintain accurate and detailed written supervision records and complete direct observations with support staff.

Assisting individuals with toileting, washing, dressing, personal hygiene and mobility according to their needs. To support with medication as directed by GP or consultant.

To ensure safe storage of medication and keep accurate records in line with the medication policy and in accordance with agreed support plans and liaise with the appropriate medical professionals as necessary.
