Community Reablement
Active support to help people regain independence within their own home.
Our Offer
MioCare’s Community Reablement Service offers planned, short-term intensive assistance to people in their own home, helping them to regain the skills needed to live as independently as possible and increase confidence in their own abilities.
Up to six weeks of support is delivered by our team of trusted assessors and support workers with specialist skills in reablement, who work closely alongside MioCare colleagues and NHS colleagues from local, integrated and connected services.

Residents benefitting from our service include people needing a short period of support. Our support can prevent hospital admissions, and enables people to remain in their own homes, promoting independence and preventing key daily skills becoming more difficult or lost altogether.
